An employee was driving a tractor trailer and exiting the employer's facility when the tractor trailer was struck by a train at a rail road crossing. The employee suffered a brain bleed, back fracture and other injuries.

An employee was driving a flat bed from the south and collided with a forklift truck travelling east in a 3-way stop intersection. The employee in the flat bed was going to make a left hand turn and did not see the forklift truck. Due to the sudden stop, the employee's leg contacted the steel in the cab resulting in a fractured leg.

On or about May 31, 2016, two employees were crossing the road in a golf cart when they were struck by a van in oncoming traffic. Both employees were admitted to the hospital.

Two employees were driving a company golf cart across an intersection when an automobile ran the stop sign and struck the cart. One of the employees sustained five broken ribs and a broken collarbone, while the second employee suffered knee bruises and a strained neck.

The employee was driving a company vehicle and making a left-hand turn when he was rear-ended by another vehicle. The employee was transported to the hospital with neck and back injuries.

An employee was walking on a ramp when her ankle rolled off the ramp and she fell forward off the ramp, landing on her right knee and right shoulder. She sustained fractures to her left ankle, right knee, and right shoulder.

An employee and two volunteers were constructing a fence at a jobsite when they were victims of an armed robbery. The employee suffered bullet wounds to the his right forearm, left little and ring fingers, lower abdomen, chest, neck, and cheek, resulting in hospitalization and multiple surgeries. One of the volunteers also suffered gunshot injuries and was treated and released.

An employee was walking toward the gym when she fell and and struck a bench. The employee was hospitalized with fractured ribs.

An employee fell from ladder to the ground and sustained multiple injuries. He was climbing from a stairwell platform to access the roof hatch to assess the HVAC unit. The distance from the platform to the roof is 12 feet. As he was climbing through the roof hatch the ladder slid from its position and the employee fell backward, head first onto the ladder that had become wedged between the stairwell walls. His left leg/boot became caught in one of the ladder rungs and he was suspended from the ladder. He managed to free his left foot before landing on the stairwell platform. He was found on the floor, lying on his back with a portion of the ladder on top of him and experienced pain in his left shoulder and left arm.

An employee was putting glasses away after their shift at the bar. A glass fell from the shelf and struck the employee's right wrist. The glass broke and lacerated an artery, two tendons, and a nerve in the wrist. The employee was hospitalized.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
