An employee was using a pressure washer to clean a sump pump. As he was standing up, the pressurized stream of water struck him, resulting in a laceration to his right arm that required surgery and an abrasion to his abdomen. The employee was hospitalized.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

Weldship Corporation

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

Two employees were moving a railcar (plastic pellet hopper car) using a mobile railcar mover. One employee was the engineer in control of the railcar mover and the injured employee was a switchman. The railcar mover went forward and the hopper railcar struck the injured employee, resulting in a severe crushing injury to the employee's right arm.

An employee was opening a filter housing in the potassium chloride production area and got sprayed with a hot solution of potassium chloride brine. The employee sustained thermal burns to their right hand and legs.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.
