An employee was moving a metal manifold with a jib hoist. The employee lowered the manifold onto the cart and was using both hands to position the manifold. The pendant struck the metal manifold and hoist lifting hook and broke the back cover off which exposed the live electrical terminals. The terminals then contacted the metal and shocked the employee resulting in a burn to the back of their left leg. The employee was hospitalized.

An employee was operating a press to manufacture parts. The employee activated the press with a two-hand control and then reached into the area where the part was located before the ram returned to the top. The employee's left ring finger was caught between the guard and the plate, resulting in a fingertip amputation without bone loss.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was cutting extruded aluminum framing material using a horizontal band saw. She went to grab a rag in the machine when the rag contacted the blade and pulled her hand in toward the blade, resulting in a partial amputation of the right index finger.

An employee was working to clear a rotary valve on a pugmill. Two fingertips were amputated by the rotary valve.

An employee was climbing an 8-foot ladder to clean windows on a house. The employee was reaching to the right when they lost their balance and slipped. The employee fell and their foot became caught in the ladder rungs, causing both the employee and the ladder to fall to the ground. The employee sustained a fracture to the right leg requiring hospitalization and surgery.

An employee was offloading material using a forklift when the hoisted material began shifting. The employee went to manually stabilize the load and their right middle fingertip was pinched and amputated.

An employee was packing candles on a line. They reached down to pick up a 12-pack of candles when their right little finger was caught in the conveyor belt, resulting in an amputation at the distal joint.

An employee was closing a door when the door closed on their right index finger, resulting in a fingertip amputation.

An employee was walking into the motor control center (MCC) room when his right ring finger was caught in the hinge of a doorway. He sustained an open phalanx fracture, which resulted in a partial amputation above the first knuckle.

An employee was changing the spacing on a telehandler's forks. A fork slipped, and the employee's left index finger was caught between it and the mast. The fingertip was medically amputated at the first knuckle.

An employee was pulling down a broken skid with a forklift. When the employee backed up the forklift to get the forks out of the skid he pulled down, he contacted the forks of another parked forklift, fracturing both of his legs. He was hospitalized.

An employee was carrying cups back to the kitchen when her foot got caught on a cart and she fell face-first. During the fall, a piece of glass from a cup cut the inside of her mouth, severing an artery. She also sustained a laceration on her lower lip. The employee was hospitalized.
