An employee was emptying a hopper on a forklift when his hand slipped from the release lever. His finger got caught between the forks and the hopper. His left index fingertip was pinched off, requiring surgical removal to the first knuckle.

Dal-Tile, Inc.

An employee entered the palletizer equipment to correct the alignment of boxed tile. The equipment restarted and contacted the employee's left leg causing a puncture wound.

TForce Freight

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

Liveo Research, Inc.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

Mohawk Industries, Inc.

An employee was operating a press when the timing belt came loose. The machine was turned off. As the employee began to adjust the belt, the pulley on the table moved and his right middle finger was caught between the belt and pulley and amputated.

INTERCERAMIC INC.

An employee was operating a dryer kiln when an alarm went off indicating that one of the two limit switches of the centering device control was not functioning properly. In order to reach the centering device control limit switch, the employee used a rod and kneeled down stretching his body/hand forward between the dryer exit and a column of an unloader machine. The employee's body was caught between the exiting tile cart and the column of the unloader machine resulting in fractured ribs.

He was hospitalized with a laceration on his forehead and a pelvic fracture on his right side.

An employee was cutting a piece of metal rod with a metal cutting machine. His right middle finger became caught between the rod and the machine's table. The employee's fingertip was partially amputated.
