An employee was operating a forklift when the unit lost power. The forklift stalled and then rolled back towards some barriers posted in front of the electrical box. The employee stuck his foot outside of the driver's compartment as the forklift approached the barrier. His foot was crushed between the barrier and the forklift.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

An employee was walking in an aisle in a warehouse when they were struck by a powered industrial truck (PIT). The employee was knocked down and part of their right foot was run over. The third toe was medically amputated to the first joint.
