On August 12, 2025, an employee was loading shingles onto the roof of a new-construction home when be began to suffer from heat exhaustion and dehydration.

An employee delivered roofing materials to a residential job site. While cutting open a pallet of rolls at the jobsite, a roll of valley metal fell and struck the employee's left shin just above the ankle, resulting in a severe cut and a lacerated tendon. The employee was hospitalized for surgery.

An employee climbed up a ladder and took two steps onto the roof. He then slipped on the roof that had a synthetic underlayment on it and fell to the ground below. The employee landed on his feet and sustained a fractured right ankle and a sprained left foot.

An employee was on the roof of a delivery location when they fell through a skylight to the ground below. The employee sustained a fractured right hip, a separated shoulder, and lacerations to his face.

An employee was delivering supplies to a roof when they began showing signs of heat exhaustion, resulting in hospitalization.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

Employees were at the trucks to leave for lunch. The injured employee was putting the cone back after the crew leader pulled the truck forward from the designated worker parking area. The injured employee then ran back to the truck. As he went to get in the truck, he said the upper thigh area of his leg was hurting. He laid on the ground outside the truck and crew leader could see a bump/bulge in the the injured employee's upper thigh area below his hip. The employee sustained a fractured femur and required surgery.

An employee was helping a contractor unload a delivery of two new dock plates from a trailer onto a forklift. The employee s left index and middle fingers were pinched by the dock plates, resulting in amputations to both fingers above the first joint.

An employee was helping to load a customer's truck. The employee stepped toward a forklift to get paperwork from the driver. The employee's right lower leg and foot were run over by the back tire of the forklift. The employee was hospitalized with fractures to his lower right leg.

An employee was moving wet bundles of shingles on a truck bed, when one of the bundles slipped out of his hands. He tripped over a pallet, fell off the bed of the truck, and landed on his left side. The employee sustained fractures to three ribs, and a displaced iliac fracture.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
