A forklift backed into, and partially onto, a temporary employee's legs. It struck and injured her right calf and her left leg near the ankle sustained a severe gash. She was hospitalized with fractures and a partial degloving of the right leg.

A temporary employee was putting a choker tube on a piler. A C-door was lowered onto their right foot. The employee's foot was crushed.

A temporary employee was setting up a deck panel form. The temporary employee was working with two other employees to make the rebar cage fit in order to close the mold for production. A chain was used to pull or keep the cage in place. A short metal form wall fell and pinned the temporary employee's leg/ankle against the form base. He sustained a broken right fibula.

An employee was moving inventory when he was pinned between his forklift and a rack. He suffered a broken rib and a punctured lung.

An employee was inspecting pieces as they came out of a machine. He went to pull apart the pieces. The punch came down and injured his fingers, resulting in amputations to his pinky finger as well as his middle and ring fingertips.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was fixing a broken purlin at a worksite when they fell from the framing to the concrete floor approximately 19 feet below. The employee sustained brain bleeding and fractures to their face, vertebrae, and wrists.

Employees were removing scaffolding that was used to repair an ion exchange tank. The line was undergoing the regen process when a piece of scaffold struck a PVC pipe that transported hydrochloric acid (HCl). The injured employee was placing material inside a scaffold rack when he was sprayed in the face with HCl and sustained chemical burns to his eyes, face, chest, right arm, and left leg.

An employee was moving a ramming machine to a staging area. While using the controls on the side of the machine, there was a sudden heavy rainfall causing the equipment to slide. The ramming machine tipped over and caught the employee against the ground. The employee sustained a fracture to the right side of their pelvis and was hospitalized.

The injured employee was training a new employee on a dough machine. She went to show where the batter comes from and her right index fingertip was cut. The employee sustained an amputation to the fingertip. The machine was not guarded at the time of the incident.

An employee was operating a roller machine on a dirt slope when the machine hit a bump and fell onto it's side. While trying to get free of the falling machine, the employee's legs were pinned by the top bar of the cage. The employee was hospitalized and his left leg was amputated. He also sustained fractures to a hand and finger.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.
