An employee was using a handheld circular saw to rip cut a short piece of wood stock. The injured employee was positioned close behind the circular saw, helping to hold the short 2 X 6 wood stock. The blade became bound, causing the circular saw to kick back. The blade struck and amputated the injured employee's left-hand thumb, requiring surgery.

LRE FOUNDATION REPAIR, LLC

The injured employee was holding a board that was being cut by another employee. The board kicked back, causing the saw to amputate the employee's left little finger and lacerate their left hand, resulting in nerve damage.

Fenson Contracting, LLC

On September 26, 2023, an employee was working in a trench box while a co-worker was lowering a piece of plastic pipe by hand using a rope. The pipe swung and struck the employee's hand, fracturing their right thumb and middle and ring fingers. The employee was hospitalized.

Two employees were using a handheld banding tool to make banding tape for tent set-ups. One employee was holding the banding tool and the injured employee was looping a metal clip around the tape. The tool closed while the injured employee's hand was near the banding mechanism. The banding tape caught the employee's right ring finger as it tightened, resulting in a partial amputation.

An employee was holding a branch against a log on the ground to make a wedge while another employee was using a chainsaw to cut the wedge. The saw slipped and struck the employee holding the branch, resulting in a laceration to the right wrist.

RG Professional Carpenter, Inc.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was on a 6-foot ladder, working to nail a 2-by-4 on a hotel carport ceiling. He lost his balance and his muddy boots slipped. He then fell and his foot lodged in the ladder. His foot was broken in two places and he was hospitalized.

An employee was installing a ceiling joist for a residential home when they fell from a framing structure and landed on the ground 8 feet below. The employee's right arm was fractured.

On September 16, 2025, a employee was moving a dolly with seven drywall sheets. The wheel of the dolly got stuck. The material tipped over and fell onto the employee's left foot, fracturing their tibia.

An employee was installing 80-foot-long trusses onto a 150-foot by 80-foot post-frame building. The employee worked from the north lift and then on the trusses about 20 feet up to install bracing. While installing diagonal bracing on the top chord, the trusses collapsed, causing the employee to fall approximately 20 feet to the concrete and be struck by the falling trusses. The employee was hospitalized with a fractured pelvis and a concussion.

GS II Building Products, Inc.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.
