An employee was in the mold maintenance area unloading an approximately 4 ft. by 4 ft. steel mold from the transport trailer with a crane. He stepped up on the trailer (approximately 29 inches high) and as he was stepping down from the trailer, he slipped and fell to the floor fracturing his left elbow in multiple places. The employee was admitted to the hospital for surgery to repair the fractures.

An employee working at a loading dock entered the back of a truck. The truck pulled away causing the employee to fall out of the truck. The employee sustained a concussion, left forehead hematoma, broken left shoulder, and broken left heal.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

FEDEX Ground Package System, Inc.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

SRM Concrete

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

U.S. Postal Service - Los Angeles P&DC

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

An employee was using a forklift to dump metal into a furnace. The furnace exploded, causing burns to multiple parts of the employee's body. The employee was hospitalized.

An employee was preparing to bend a steel tube by loading it into a computer numerical control (CNC) bender's clamping jaws. Her right middle fingertip was caught in the clamping saws, resulting in an amputation.

An employee was walking to the breakroom, tripped over a container tote, and fell to the concrete floor. The employee sustained a left ankle fracture.

An employee was running a coil into the die when his hand slipped and contacted the cut-off hole of the die. The employee sustained a partial traumatic transphalangeal amputation of the left middle finger.

An employee was entering the building when he missed a 4-inch ledge and fell to the floor, resulting in a tear to the soleus muscle in the right leg.

United States Postal Service

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

GXO Logistics

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
