Final narrative
An employee was slicing cucumbers on the deli slicer. While attempting to retrieve a piece of cucumber, the employee's right hand made contact with the blade, resulting in a right fingertip and fingernail amputation.
On October 15, 2018, at approximately 7:25 a.m., an employee was walking into a freezer when the employee slipped on some cardboard and fell to the floor, breaking the right arm. The employee was hospitalized and had surgery.
An employee was working at a cash register when they tripped over a power strip. The employee fell, suffering fractures to the right shoulder, right knee, and a rib on the left side.
An employee was operating a strip saw. As the employee shut off the saw, their right index and middle fingertips were amputated by the blade before it came to a complete stop.
