The employee suffered amputations of the middle, ring, and little fingers at the middle knuckle and the loss of skin on the index finger, resulting in hospitalization and surgery.

An employee was assisting water treatment plant personnel with changing a flange when their finger got caught between the two pieces. The employee sustained a finger amputation.

The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.
