An employee was working at the loading dock checking a pallet when she tripped over a load bar and fell, breaking her right hip (femur). She was hospitalized for surgery.

An employee was walking to the break room when she tripped and fell on a piece of plastic that was sticking out from a pallet, fracturing her left elbow and tearing her shoulder tendons.
An employee was bent over straightening a shelf of merchandise when he stood up and fell, striking his head and neck on the floor. He fractured his second and third vertebrae.
Four employees were dumping fixtures from the platform on a forklift into the dumpster. An employee climbed up the side steps of the dumpster to pull fixtures from the platform and fell to the ground fracturing both shoulders, a knee and the nose.
