An employee was organizing an area full of part buggies and pushing them to their proper locations. As the employee was pushing a cart, the front side struck another buggy. When this happened, it caused wood stiles inside the buggy to shift, crushing her right middle finger between the stiles and the handle of the buggy and resulting in a fingertip amputation.

An employee was profiling door skins on a table saw when his right-hand middle finger and palm made contact with the saw's blade. His middle fingertip was amputated, and his palm was lacerated.

An employee was operating a forklift and unloading a truck. After the truck was unloaded, the employee got off the forklift and proceeded to adjust the dock plate with a hook tool. The dock plate slipped and the employee went to adjust the plate with their hand when their left middle finger got caught. Their finger was amputated above the first joint.

An employee was rotating a carbide granulator blade when his finger was caught between the blade and a stationary part of the machine. The employee's right middle finger was partially amputated and they suffered an avulsion to the right ring fingertip.

An employee was using a CNC metal lathe machine in manual mode to check that the thread blocking tool made contact with the jaws. As the employee was manually spinning the spindle, his index fingertip was crushed between the spindle's jaws and the grooving insert, resulting in a fingertip amputation.

During a material handling operation, an employee's fingertip was caught under an air conditioning unit as the unit was lowered onto a cart. The employee's fingertip was amputated.

An employee was removing a bolt from a tractor hub when their right index finger was caught between the hub and the frame. The employee's fingertip was crushed and the employee was hospitalized.

The rollers activated, which processed material through the machine, caught the employee s glove, and pulled their hand toward the cutting head of the molder. The employee sustained lacerations to their right middle and ring fingers and an amputation to the distal phalanx of the middle finger.

On August 11, 2025, an employee was operating a clamping machine that used two pneumatic cylinders to clamp boards together for gluing. While he was positioning a board, the control was activated and the clamp on one side of the machine closed. A fingertip on his right hand was caught between the clamp and the board. He suffered an amputation to the fingertip (involving a torn fingernail and surrounding skin) as well as a fracture to the last joint of the finger.

On July 21, 2025, an employee was working from a 10-foot step ladder while installing a 3-by-5-foot glass window pane on the outside of a new home under construction. The window fell from the frame and the employee went to catch it when the ladder slipped. The employee fell 5 feet to the ground and landed on small pieces of concrete left over from the foundation pour. The employee was hospitalized with a dislocated elbow and fractured wrist that required surgery.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.
