Final narrative
The employee was walking, tripped over a wooden pallet, and fell breaking his hip.
On or about March 10, 2015, at 2:30 p.m., two employees got struck by a drilling motor and a cart. While pushing the cart from the outside of the building to the inside, the cart tipped and the drilling motor fell off. The cart struck the first employee in the torso area and pinned him underneath. The cart struck the second employee when it tipped and propelled to the side. The second employee sustained a forehead contusion, fractured left ankle, right ear laceration, and fractured facial bone. Both employees were taken to the hospital.
An employee arrived at work and was walking into the store through the parking lot. The employee tripped over a curb and fell to the ground, sustaining a femur fracture.
An employee was ascending an 8-foot aluminum extension ladder to access and replace a hypochlorite pump located on top of a 7-foot chemical tank. When he reached approximately 5 feet on the ladder, the ladder shifted and twisted to the right. The employee fell from the ladder and landed on his right side.
