A football player was running a route during football practice. Upon cutting and changing direction, they felt pain in their lower back. The employee was hospitalized with a herniated disc in their lumbar region.

An employee was participating in training. As they were jogging, they sustained a muscle injury.

On May 14, 2022, an employee was strapping their truck and a beam started to fall down. The employee turned and started to run away and injured his quadriceps. He was hospitalized with a muscular injury to the left leg.

An employee had chest pain after chasing a patient who was attempting to leave the unit.

An employee felt faint during a mandatory physical training run. He sat down on the running trail and started going in and out of consciousness. The employee was hospitalized.

An employee was walking on a ramp when her ankle rolled off the ramp and she fell forward off the ramp, landing on her right knee and right shoulder. She sustained fractures to her left ankle, right knee, and right shoulder.

An employee and two volunteers were constructing a fence at a jobsite when they were victims of an armed robbery. The employee suffered bullet wounds to the his right forearm, left little and ring fingers, lower abdomen, chest, neck, and cheek, resulting in hospitalization and multiple surgeries. One of the volunteers also suffered gunshot injuries and was treated and released.

An employee was walking toward the gym when she fell and and struck a bench. The employee was hospitalized with fractured ribs.

An employee fell from ladder to the ground and sustained multiple injuries. He was climbing from a stairwell platform to access the roof hatch to assess the HVAC unit. The distance from the platform to the roof is 12 feet. As he was climbing through the roof hatch the ladder slid from its position and the employee fell backward, head first onto the ladder that had become wedged between the stairwell walls. His left leg/boot became caught in one of the ladder rungs and he was suspended from the ladder. He managed to free his left foot before landing on the stairwell platform. He was found on the floor, lying on his back with a portion of the ladder on top of him and experienced pain in his left shoulder and left arm.

An employee was putting glasses away after their shift at the bar. A glass fell from the shelf and struck the employee's right wrist. The glass broke and lacerated an artery, two tendons, and a nerve in the wrist. The employee was hospitalized.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
