A driver was checking the oil in a dump truck while standing on the bumper, approximately two feet high. The employee's feet slipped off the truck's bumper and the employee fell backwards hitting his head against the ground. The employee suffered a concussion and neck pain.

An employee was climbing a step ladder while carrying a 3-foot piece of conduit. As he went to reposition his feet on the ladder by pivoting, he slipped and fell from the third rung of a 6-foot ladder. The employee sustained fractures to the left femur, right elbow, and right ring finger.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

FEDEX Ground Package System, Inc.

An employee stopped a belt and was going to check it for missing packages. She fell backward 3-4 feet from an elevated platform and sustained a broken right arm.

SRM Concrete

An employee had just finished a routine concrete pour and was ascending the ladder to clean the concrete mixer truck. The employee lost their grip and fell approximately 2-3 feet, contacting the truck's bumper. The employee suffered rib fractures and a punctured lung.

U.S. Postal Service - Los Angeles P&DC

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

QB Corporation

An employee was performing troubleshooting and maintenance on one of the generators that feed the RF line for glue-laminated timber (glulam) beam production. He was reattaching the outer panel of the generator. A circuit was completed and the employee was electrocuted by the generator and sustained burns. The employee was hospitalized.

Oncor Electric Delivery Company, LLC

An employee was utilizing a utility task vehicle (UTV) to patrol a 138 kV transmission line in a right-of-way. The UTV drove into a washout hole that was approximately 6 feet deep and obstructed by vegetation. The employee sustained a dislocated shoulder, a fractured left humerus, back pain, and a strained ankle. He was hospitalized.

Central Maine Power Company

A crew of employees were working to replace a utility pole that was broken by a motor vehicle accident. As the crew was removing equipment from the broken pole, part of the pole broke, fell on the injured employee, and rolled over him. The employee sustained two broken shoulders, multiple broken ribs, and a damaged lung that required hospitalization and surgery.

Ameren Illinois - Sparta Operating Center

An Ameren lineman was making secondary voltage connections in a pad-mounted residential feed transformer. The lineman contacted an energized lead bushing, sustained an electrical shock, and was hospitalized.

An employee was checking voltage on a meter. There was an arc flash, and the employee suffered burns to both hands.

GS II Building Products, Inc.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

US Battery Manufacturing Company, Inc

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.
