On October 9, 2015, a shuttle driver fell out of a moving vehicle while attempting to rid the vehicle of a bug. No seat belt was worn at the time. The employee suffered a contusion to the right leg that required a medical procedure. The employee was hospitalized.
On March 27, 2023, an employee fell from the back of a traffic truck while it was performing a U-turn. The employee landed on the ground and sustained injury to the neck and back.
An employee was on a 20-foot ladder cleaning tile on the side of a building. The ladder slipped and he fell to the ground. He landed on his feet, resulting in hospitalization with fractures to both feet.
An employee was putting away stock in the aisles and began driving his order picker down the aisle with a pallet of items loaded on the forks.
