An employee was servicing an onsite trash container when he lost his balance and fell from the container to the ground. He is leg was broken just above the knee.

An employee suffered heat stress while dumping trash cans on a garbage truck, becoming lightheaded and losing consciousness. The employee then fell and struck their head on the truck frame, suffering a head laceration that required stitches. The employee was hospitalized.

An employee was delivering a forklift when part of it broke off at the weld and the forklift fell 3 feet. The employee then hit his head on the bar support for the roll-over protection and suffered a laceration that required hospitalization.

An employee was bitten by a copperhead snake while picking up loose material.

An employee was on a service call, replacing a sensor on a fleet truck. He stood on a battery box, and put his right leg on a cylinder to help hold his balance. His hands were occupied with hand tools in a position above his head. His left foot slipped, putting all of his body weight on his right foot and resulting in a fracture below the knee.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee was performing maintenance on a hopper. The employee's hand was caught in a cylinder pivot point area when the pneumatic cylinder released trapped air. The cylinder crushed the employee's left little finger, causing the partial amputation of the fingertip.

An employee was opening a door to a semi trailer at the facility and his hand was caught between the door and another parked semi. The employee's left index finger was fractured and thumb muscles were injured. The employee was hospitalized and required surgery.

An employee was removing residual meat particles from the sprocket of the crossover conveyor belt in the packing area. While doing so, the conveyor started and his fingers were caught between the metal framework and the belt, resulting in amputations to the middle and ring fingers on the left hand as well as skin damage to the right hand.
