An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee had been carrying two 5-gallon buckets and walking to return the buckets to a drying rack area. While going to hang the buckets, the employee tripped over a large scale ramp and struck his face against the wall. The employee was hospitalized with facial fractures and required surgery.

While walking out of a building, an employee stepped on the corner of a sanitizing footbath, lost their balance, and fell forward. In bracing for their fall, the employee suffered fractures to the left humerus and right wrist. The employee was hospitalized.

A temporary employee was in training and utilizing an electric riding pallet jack when it drove up onto another pallet jack. The employee went to move it when the pallet jack ran over his feet. The employee sustained an injury to his left leg/foot and required surgery.

An employee was on an extension ladder working to put a tarp on the roof. He was zip-tying a tarp to a pipe when the ladder slipped. The employee fell and struck his head on the floor. He was bleeding from his head and lost consciousness. He also sustained a broken arm or hand.

An employee was operating a tear out machine when his right arm was caught in the machine. The employee was hospitalized with a fractured arm and required surgery.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
