An employee was helping a forklift lift a metal brace that was frozen to the ground when the brace became free and popped up, hitting and breaking the employee's ankle.

The injured employee was standing on a form/bed where concrete is poured and was removing concrete from the form(s). Another employee indicated through hand signals to the injured employee that the chocks had not been removed from the strands. The employee jumped down from the bed to ground level. The bed then started moving toward him. He went to jump over the drive wheel to get out of the way of the bed, but his foot got caught in the drive wheel. The employee sustained a severe laceration and crush injuries to the foot that required surgery.

An employee was on the frame of a concrete casting machine as it traveled down the tracks. His left foot got caught between the rail, the machine frame, and the operator's platform, resulting in a crush injury.

An employee was training a new operator to transverse (cross) a casting machine. The machine's wheels were lowered to the transverse track while the employee was reaching under the wheel assembly to remove debris from the track. The wheels crushed the fingers of his right hand, causing amputations to the index, middle, and ring fingertips.

An employee was cleaning a spray paint hopper bucket with a pressure washer. The employee reached down to adjust the position of the bucket with his left hand when his left index finger contacted the water from the pressure washer, resulting in a laceration that required hospitalization.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

Red Dot Corporation

An employee was attempting to lift a steel beam from a horizontal, stacked position to a vertical position using a crane. They had placed two hooks on the east side of the beam on the flange. As the employee was hoisting the crane, the beam began to shift and pulled the employee forward onto the stack. When the material shifted, the hooks released and the material fell, pinning the employee between the two beams. The employee was hospitalized with soft tissue contusions on their proximal right thigh and interior left thigh.

An employee was standing on the surface of a steel beam about two feet wide. He slipped and fell backward, landing on the concrete ground about 20 feet below. The employee sustained a broken pelvis.

K Constructor, LLC

An employee was unloading a rebar delivery from a flatbed, 19-wheel trailer. The employee rigged the load with wire rope chokers. The load was picked up 3-4 feet above the trailer deck by a crane. The load began to swing and the employee grabbed onto it to prevent it from being pushed off the trailer deck. The crane continued to lift the load, and the employee let go so the crane did not lift him to the third story. The employee fell approximately 12 feet to the ground below, resulting in a back injury and fractured vertebrae.

An employee was attaching a steel angle plate weighing (approximately 200 pounds) to the end of a structural member (5,000 pounds) and was supported by dunnage. The dunnage was a crane mat (11 inches tall) with a 4x6 board on top of it and it supported five identical members. The main piece was 30.75 feet long, 37.25 feet tall, and 1 food wide. As the employee slid the piece of metal on top of the member to begin attaching it, the entire member began to tip over. The employee went to stop the main piece from tipping over and it landed on his ankle, fracturing it.

An employee fell while exiting a trailer. The employee landed on their left side on the pavement, suffering multiple broken ribs on the left side. The employee was hospitalized.

Two employees were setting up a mandrel in the spindle of a machine. The mandrel was side-shifted while an employee's hand was on the mast of a powered industrial truck. The employee suffered crushing injuries that resulted in amputations to the middle and ring fingertips.

An employee had been loading a double deck press. The employee saw a mold had been sent in with the C-hook still attached on the belly bar. As he went to remove the hook from the mold, his right thumb got caught on the C-hook. The employee's thumb was partially amputated.

An employee was operating a roller and paving a small pathway next to a pavilion. The ground was on a slight pitch, causing him to reach up toward the roll cage to stabilize himself. His right fifth finger was pinched between the roll cage and the rafter of the pavilion. The employee sustained a partial degloving injury with partial traumatic amputation.

An employee was delivering home heating fuel when bees came from the ground and stung the employee's face, neck, torso, and hands. The employee proceeded to shut off the oil flow before losing consciousness. They were hospitalized due to an anaphylactic reaction.
