An employee was walking to pick up fall protection equipment when a 4'x8' sheet of 3/4-inch plywood fell approximately 9 feet and struck them. They sustained a head laceration and compressed discs of cervical vertebrae 4-6.

On September 7, 2021, an employee had adjusted a chain link fence and was standing next to the fence. A forklift moved towards him and crushed his right hand against a fence pole. The employee was hospitalized.

An employee had landed a load of glass in an "A"-style crate. The crane rigging caught on the bottom of the crate, causing it to tip over and land on the employee's left side and ankle. The employee's ankle was broken.

An employee was working with a hammer to remove wall ties when a drill was dropped from the next level up, striking him on his hard hat and causing neck and shoulder injuries.

An employee was walking beside a 26 ft. electric scissor lift when his right foot slipped and was run over by the left wheel of the scissor lift. The employee was hospitalized for fractured bones in his foot.

An employee was walking to their truck to look for a lid. When they stepped between the truck and a dock plate, they sustained a knee injury that required surgery.

An employee was standing on plant bed timbers, using a powered hand trimmer to trim the shrubs in the plant bed. The timber gave way, breaking into several pieces. The employee lost balance and fell with the trimmer in his hand. He suffered a severe laceration to the right wrist and hand.

A mail carrier slipped on ice and sustained a strain that required hospitalization.

An employee was loading phosphoric acid onto a rail car and preparing to secure the rail car for transport. While repositioning the loadout spout to secure it back to the drip pan holster on the dock, the spout contacted the handrail of the rail car, causing the employee to lose balance. The employee stepped back and their right foot entered the opening for the rail car, causing him to twist/pivot on his left leg. The employee's lower left leg was fractured.

On November 14, 2023, an employee was receiving a delivery from a truck when some of the merchandise fell from the truck s lift and struck him. He then fell to the ground and his head struck the sidewalk, resulting in an injury that required hospitalization.

An employee was cutting cabinet trim using a table saw when four fingers on his left hand were severed, resulting in hospitalization and amputation.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was installing insulation in a wall when she tripped on a hole (12" deep) and fell, hitting her head on the concrete floor. The employee sustained fractures to her leg and wrist, and lacerations to her head. The employee was hospitalized.

An employee was loading a 4-inch wooden door into a pickup truck when he felt a pop in his right side behind his shoulder blade. He was hospitalized later that day and underwent surgery, having suffered a collapsed right lung.

An employee was operating a power drill when it slipped. The drill bit punctured the employee's hand and they were hospitalized.

An employee was helping to lift the grating from a floor draining system when the grating slipped and landed on his hand, resulting in the amputation of his right middle finger at the first joint.

A casting machine jammed. An employee's hand was caught in the machine, where a belt line caught and amputated the tip of his finger.

During a workshop meeting in a hotel, an employee heard a drilling noise, so he walked outside to see what it was. An explosion occurred (possible gas line) and his face, ear, and hair were burned. He also fell and sustained a pelvic fracture.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
