Final narrative
An employee was unbolting screws that secured a steel flange to a collar support at the end of a hydraulic cylinder. After a coworker separated the collar from the flange using a pry bar, the 200- to 300-pound flange fell and crushed the employee's little finger.
Employees were relocating a blow molding machine at a client site. The machine tipped over and pinned an employee, causing a hernia and a laceration on the right side of their abdomen.
