An employee was operating a company-owned vehicle on a roadway inside a worksite. As the employee was driving downhill, the vehicle brakes failed and the vehicle struck an embankment at the bottom of the hill. The employee sustained a vertebra fracture in his back and was hospitalized.

An employee was descending a 4-step maintenance stand when she missed the bottom step and fell to the hangar floor. The employee suffered a left hip fracture.

An employee was performing duties as an expeditor. After opening the dock door, the employee scanned the barcode on the door of the truck and placed one foot on the truck and one foot on the dock. The truck drove out of the stall, causing the employee to fall 4 feet off the dock onto the concrete. The employee sustained fractures to the right side of the pelvis, elbow, and a left ring fingertip as well as injuries to the right wrist and bruising to the back and stomach.

On 08/10/18, at approximately 2:00 AM, a maintenance employee was performing repairs to an oxygen line that was part of a plant's oxygen air system. The employee repaired the hose, tightened the clamp on the hose, and instructed another employee to turn the oxygen back on. When the oxygen was turned back on, the hose exploded in the maintenance employee's hands. The employee was hospitalized with burns to both hands.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.
