On November 9, 2023, an employee was removing plywood from the west face of a basement level buck hoist to prepare to dismantle one of the cabs. When the employee pulled on the plywood, it fell and pulled a rope that partially amputated the employee's thumb.

Systems MEC LLC

An employee was holding a rigging line and assisting in the removal of an old light pole from its base. When the load was lifted by a forklift, it shifted. The tag line lifted the employee off the ground and propelled him through the air. The employee landed on the ground and sustained back fractures.

An employee was performing maintenance on a bucket truck when his right hand became caught in some rope, causing amputations to three fingers down to the first knuckle.

The employee's left hand, little finger, and thumb were caught in the wire rope lanyard, resulting in a medical amputation of the left little finger as well as lacerations to the left thumb.

An employee was using a crank and cable to lower a light tower due to windy conditions. The light shaft became hung up and released the tension in the cable, which wrapped around the employee's left hand, resulting in amputations of three fingers.

An employee was trimming tree limbs from a bucket truck. The employee fell from the bucket to the ground 16 feet below, suffering a concussion and a broken rib.

Two employees were in a drive terminal, pulling a new fuel line for an EPU to provide tertiary power for a ski lift. After unspooling the fuel line, one employee took a step backward and fell through the skinning (the floor of the terminal) to the ground about 12-15 feet below. The employee suffered a dislocated ankle and fractures to the lower tibia and fibula.

An employee was doing a sweep of a mountain bike trail when their mountain bike's front tire slipped. The employee fell from the bike to the ground and suffered a broken pelvis. The employee was hospitalized.

An employee was kneeling, lighting the pilot for a gas range, when gas that had leaked from a quick coupler at the back of the range ignited. The employee suffered serious burns to both arms and her neck.

An employee was working to fix the forks on a forklift when the fork(s) fell on her toes. The second and third toes on her right foot were fractured, lacerated, and required stitches.

Milton Rents

An employee was delivering a 65-foot boom lift to a rental customer's construction site. The employee set up the ramp truck in the parking lot adjacent to the construction site. He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
