An employee was positioned at the rear of a 60 ton truck crane. The crane's outrigger jacks were raised in preparation for re-positioning the crane. As the jacks were raised the crane traveled backward approximately 5 feet. As a result, the employee's right leg was caught between the rear of the 60 ton crane and the outrigger mats being utilized by an adjacent 240 ton crane. The employee suffered a puncture wound and fracture to the right knee requiring corrective surgery.

On April 5, 2024, at around 8:45 PM, a 100-ton crane was being towed when the crane's brakes overheated and caught fire. While putting out the fire, a tire caught fire and exploded. Shrapnel and debris struck an employee's right leg resulting in lacerations and avulsions to the leg. They also sustained contusions to their left foot.

An employee was assisting with disassembling a 500-ton crane. The injured employee was pushing an outrigger while another employee pulled from the other side to stow the base of the outrigger. The injured employee's left ring finger became caught in the rail system, requiring surgical amputation at the first knuckle.

An employee had just mounted the all-terrain crane deck using the installed stairway and was egressing across the crane deck to enter the operator's cab when they lost footing and fell off the upper cab's walkway. The employee fell down and away from the crane approximately 6 feet to the ground. The employee sustained an L1 vertebra fracture.

An employee was standing on the deck of a 4-foot float trailer helping a crew member remove pins to swing the jib on a crane. The employee fell off the float trailer and struck the ground below, resulting in a compound fracture to their left ankle.

An employee was standing next to the outrigger beam of a crane. The beam retracted and pulled the employee's hand in-between two pieces of metal, resulting in amputation of the right ring fingertip.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

An employee was maneuvering a motorized pallet jack while standing in front of it. He became pinned between the pallet jack and a steel rack cross-beam, resulting in a contusion to his chest/back and a fractured rib.

An employee was stacking empty drums into a trailer. The employee then exited the trailer, using the forks of a forklift as a step. The forklift's parking brake failed, causing the forklift to move forward and pin the employee's right leg between the forklift and the trailer. The employee's right lower leg was crushed, resulting in a fracture and a laceration.

An employee had been spraying weeds behind a school. As he was leaving the field, the employee drove his truck up to the gate, got out, and opened the gate. The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

Stevenson Crane Service, Inc.

A crew was relocating a dumpster (containing approximately 200 pounds of demolished wire) to access the uninterruptible power supply (UPS) units they were servicing. During movement of the dumpster, a section of carpet tile dislodged from the floor, causing an employee to lose his footing, slip, and fall. He landed on his right wrist, resulting in a fracture.

An employee was cutting a board using a circular saw. The saw pinched, kicked back, and cut the employee's left ring and little fingers. The employee suffered an amputation to the little finger.

Severo Ortiz Construction LLC

An employee was using an aluminum extension ladder to remove gutters at a height of approximately 20 feet. As they were removing the gutters, part of the gutter collapsed and struck the ladder, causing it to move sideways. The employee fell from the ladder and sustained fractures to ribs on the left side and their right forearm.

An employee was using an electric saw to cut a piece of wood for a fence. The saw slipped out of his hand and lacerated his left leg. He was hospitalized.

An employee was erecting a scaffold. A side bracket was not secure. The employee stepped on the bracket to test it and fell 18 feet to the concrete floor resulting in fractures to the ankles and femurs. He was not using fall protection equipment.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.
