An employee was performing housekeeping duties when she noticed a pallet stuck on conveyor rollers. She went to adjust the pallet when her left little finger became stuck between the pallet and the curbing of the conveyor, resulting in a partial fingertip amputation.

The injured employee was using a powered industrial truck (PIT) to pull empty pallets from racks. She was backing up when another employee honked their horn to alert her of their presence. The injured employee's forklift then moved forward and struck a rack. Her left foot was fractured.

On May 3, 2025, an employee was getting an item from the freezer when she tripped on a pallet that was behind her and fell, striking the ground. The employee was hospitalized with an injury to the lower right side of her body and muscle spasms.

An employee slipped while climbing a ladder on a skidder, fell to the ground, and suffered a tibia/fibula fracture. The employee was hospitalized.

He elevated the boom 4-5 feet to clear a temporary fence. While driving the boom lift off the ramp, the wheels slid to the driver's side of the ramp truck, ultimately sliding off the ramp. The boom lift fell off the side of the truck and rolled onto its side. The employee sustained a fractured clavicle, a dislocated shoulder, separated/fractured ribs, a punctured lung, and a fractured left femur. The employee required surgery.
