Final narrative
An employee was retrieving a part from an order when her foot got caught in a plastic strap, causing her to trip and fall. She fractured her femur (right leg) and was hospitalized.
Two employees were moving material that was loaded on a material cart that weighed 3,500-4,000 lbs. The material cart was on an electric pallet jack. The injured employee tried to help stop the pallet jack and was caught between the material cart and a support beam, sustaining a ruptured spleen.
While exiting a truck cab, an employee missed the last step and fell to the concrete pavement, suffering a laceration to the back of the head, as well as pain in the neck, back, and hips.
An employee was preparing for a confined space entry and got stuck between two pieces of process equipment resulting in a broken shoulder, lacerations and facial bruising.
