An employee was fixing an airline utilizing a lift. He fell over the rail as he was repairing the airline with a pair of pliers and struck a tank car below. The employee fractured his pelvis.

An employee was standing on a 6-foot ladder using a handheld grinder to grind the sidewall inside of a rail car. The guard on the grinder slipped and the grinder wheel fell off striking the employee's index finger resulting in a fingertip amputation.

An employee was neutralizing an empty railcar that had carried sodium hydroxide. While the employee was cleaning the tank, sodium hydroxide splashed on his face and arm, causing 2nd degree chemical burns.

An employee suffered from 2nd and 3rd degree burns to his chest, waist, and left hand when the shirt he was wearing caught fire while he was operating a gas-powered cutting torch to cut 8 bolts.

An employee was installing wood trim surrounding a skylight inside a home. While holding a piece of trim in it's desired location, his pneumatic nail gun discharged, sending a 3/4-inch pin nail into his left forearm.

An employee removed a safety valve on a trailer that unknowingly contained nitrogen gas. The gas released from the trailer and pushed the employee backward off a scaffold approximately 10 feet before they struck a garage door, resulting in a pelvic injury.

An employee was sitting at her workstation when a nail gun malfunctioned and shot a nail into her left leg. She suffered a puncture and blood vessel injury and was hospitalized.

On November 25, 2023, an employee was using a high-pressured water hose to perform sewer line cleaning operations. The employee's right little finger and palm were lacerated by pressurized water from the hose.

An employee was hydroblasting when they slipped and the pressurized water stream lacerated their left ankle.

An employee was working to unjam an auger screw. The screw caught his glove, resulting in a severe open fracture to his left lower forearm. He was hospitalized, requiring surgery.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
