An employee was unloading a forklift from a truck. The forklift's mast tilted and the forklift fell approximately 18" to the ground. The employee struck his head on the forklift's overhead crossbar and sidebar, causing head lacerations and a displaced fractures of the C6 and C7 vertebrae.

An employee was standing on the back of a flatbed tailgate, throwing trash into a dumpster. The employee fell off the tailgate and landed on their hip and ribs sustaining three fractured ribs and a collapsed lung.

A driver was helping unload product at a customer location. As the customer used a forklift to unload the product, an iron pipe started slipping. The employee was using a tightening rod to push the bar when his left thumb was caught between the pipe and the rod. The thumb was crushed and broken.

An employee was using a cart to move welding pipe weighing about 900 pounds. The cart tipped over and the pipe struck him in the left leg. He suffered a contusion to the upper thigh and was hospitalized due to concern about blood clots.

An employee was making a delivery at a customer's shop. He fell from a flatbed truck to the pavement and sustained a broken femur. The employee was hospitalized.

On December 13, 2023, an employee was operating a forklift. When they stepped off the forklift, it crushed their left foot between the lift and a racking system. The employee suffered a foot fracture that required hospitalization.

An employee was maneuvering a motorized pallet jack while standing in front of it. He became pinned between the pallet jack and a steel rack cross-beam, resulting in a contusion to his chest/back and a fractured rib.

An employee was stacking empty drums into a trailer. The employee then exited the trailer, using the forks of a forklift as a step. The forklift's parking brake failed, causing the forklift to move forward and pin the employee's right leg between the forklift and the trailer. The employee's right lower leg was crushed, resulting in a fracture and a laceration.

An employee had been spraying weeds behind a school. As he was leaving the field, the employee drove his truck up to the gate, got out, and opened the gate. The truck rolled forward and pinned him against the fence/gate. The gate then gave way and opened from the force of the truck. The employee's left index fingertip got caught in the fence/gate and was amputated.

An employee was using a walk-behind tugger to pull a grinder. The employee's left little finger was pinched between the handle of the tugger and the framework of a staircase. The employee's fingertip was amputated.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

A temporary employee was punched in the face by another temporary employee. He suffered a laceration to the eye.

An employee was backing up a powered industrial vehicle. Her left hand was caught between the vehicle and a rack, and the wrist was broken.

An employee was operating an extruder and performing a spool swap over from the right spindle to the left spindle. After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.

An employee was resetting a warehouse racking system utilizing an order picker. They fell approximately 5 feet from the order picker to the concrete floor and sustained fractured ribs.

After cutting a slab of beef short ribs, an employee turned to grab the pieces he had cut and his right hand contacted the saw blade. The employee sustained an amputation to his right index finger.

An employee was unloading a carpet pad from a truck when they fell from the truck dock to the concrete below, resulting in five fractured ribs and an injury to their left lung.

An employee was inspecting flares processes. The employee received burns to the front side of the body, face and arm from the flares.

An employee was removing plastic material from a production line when the machine cycled and amputated his left index, ring, and little fingers.

An employee was walking up a walkway to enter a building through the back entrance when she tripped over the lip of the cement ramp. The employee fell and sustained a fractured right hip.
