An employee was attempting to remove a cardboard corner post on a pallet. He was using his right leg as leverage on the pallet. As he was bending the corner post, it released, and the pallet slipped out from under his foot. He fell back onto the concrete floor, breaking his left hip bone.
An employee was installing a part in a press machine when her leg hit the switch. Her left pinky fingertip got caught in the machine and was partially amputated.
