On Monday, 5/9/2016, at 9:40 AM, an employee was using a pallet jack to unload materials from a vehicle with a lift gate when the load and pallet jack fell on his feet. He sustained a broken left ankle and a contusion to his right ankle, requiring hospitalization and surgery.
