On October 30, 2024, an employee was removing a water-soaked ceiling tile while on a ladder. The tile fell out of the ceiling and the employee tried to get out of the way of the falling debris. He missed a step on the 6' ladder and fell to the floor. He struck his head and sustained an abrasion on his left arm and a rupture of his left patellar tendon.

On June 8, 2023, an employee was using his right hand to close the trifold liftgate of a box truck when his left little finger got caught between sections of the gate. The employee's left little finger was partially amputated.

An employee was driving a triple electric pallet jack, traveling northbound in the main drive aisle. A smaller electric pallet jack was traveling southbound on the opposite side of the drive aisle, which is separated by a pedestrian walking aisle. The employee turned toward the inbound side of the building prior to crossing the pedestrian walkway and entering the opposing drive aisle. The two pallet jacks collided. The employee was ejected from their pallet jack, fell to the floor, and her left leg was run over by the smaller pallet jack. The employee sustained crush injuries to the lower leg and foot that required amputation of the lower leg.

An employee was moving a pallet. When he swung the pallet to the floor, a protruding nail on the pallet cut his left forearm and hand. He suffered a broken left wrist.

An employee was operating an extruder and performing a spool swap over from the right spindle to the left spindle. After rewiring the new spool, the employee's clothing got caught in the turning shaft when the left spindle started back up. The employee's left arm was pulled into the machine, resulting in a fractured humerus and lacerations to her triceps. She was hospitalized and required surgery.
