An employee was checking the oil on the reefer unit of a trailer. A third-party tractor did not see the employee and pinned the employee between the tractor and the trailer. The employee suffered broken ribs, a broken sternum, and a collapsed lung, requiring hospitalization.

An employee was exiting a trailer on a pallet jack. A pallet got caught on the dock plate, flipping the employee from the pallet jack and onto the concrete warehouse ground, hitting his head. The employee was hospitalized.

An employee slipped/tripped and pulled the handle of a pallet jack to regain his balance. The wheel of the pallet jack ran over his left foot breaking it.

On December 30, 2023, an employee was pulling a pallet into the back room when a forklift rounded the corner and ran over the employee's right foot. Three of the employee's toes were amputated.

An employee was unwrapping a pallet of empty buckets. The employee stepped backward and was struck by a powered industrial truck carrying a pallet of bucket lids, resulting in a left ankle fracture.

An employee was walking in the racking area to check parts when they were struck by an oncoming forklift. The front wheel ran over the employee's right leg, resulting in fractures to the leg and ankle.

An employee was exiting a trailer for it to be loaded when their right ankle was pinned between a piece of angle iron that was anchored to the floor and a long pallet being pushed by a powered industrial truck (PIT). The employee sustained an ankle fracture and was hospitalized.

A tow truck operator was operating his bed controls on the drivers side when another vehicle crashed into him. The employee sustained fractures to the right femur, hip, and left foot, as well as abrasions to the right knee and a laceration to the right wrist.

An employee was working to unjam an auger screw. The screw caught his glove, resulting in a severe open fracture to his left lower forearm. He was hospitalized, requiring surgery.

An employee was working in a warehouse without air conditioning on a day when the temperature outside was in the 90s. The employee developed heat stress, became ill, and was hospitalized.

An employee was walking in the middle of the track during heavy rain when he slipped on a rail transition and fell, hitting his arm on the rail. The employee was hospitalized with a fractured left arm.

An employee was scraping down the sides of a mixing machine. The machine activated and her right arm was caught in the auger, resulting in a fractured lower arm.

Two employees were conducting a roll change on a rolling mill machine using a porter bar and a counterweight sleeve. The sleeve slid back and crushed the injured employee's right index fingertip between the sleeve and the counterweight, resulting in a partial amputation.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
