An employee was using a pry bar to align a flange when the bar slipped, causing him to fall backward and fracture his left ankle. He was hospitalized for surgery.
An employee was removing supports under an auger when the auger fell and struck the employee's head. The employee suffered bleeding and swelling of the brain.
An employee was pulling on a bale of corrugated cardboard boxes that were on a conveyor. The straps on the bale failed and the employee fell to the floor. The employee was hospitalized and required surgery for a fractured femur and/or hip.
