An employee was standing behind his forklift, scanning freight when another forklift backed out of a trailer into him and injured his right leg.

An employee was exiting a yard horse/terminal tractor when he slipped off the top step and his knee contacted the edge of the metal step. The impact severed the lower patella tendon and surgery was required.

While dragging a pallet to the edge of a trailer, an employee fell off the trailer to the ground, suffering thumb and back fractures and a head laceration.

As an employee was backing out of a trailer to close it down, the bands securing a skid broke. The skid collapsed, causing the bucket of a front-end loader to shift and fall on the employee's left foot, lacerating and breaking it.

An employee was unloading an 800-pound dock plate from a trailer when the dock plate fell on him, fracturing his pelvis and requiring hospitalization.

An employee was using a crane. The hook slipped and a fitting fell onto a finger on the employee's left hand causing a fracture and laceration.

An employee was transporting an 800-pound die on a cart from the tool room. The employee was pulling the cart when it struck a metal plate on the floor. This caused the cart to tip forward and the die to slide off the cart. The die struck the employee's left foot and their second toe was amputated.

An employee was using a forklift to load pallets of product on a trailer. He exited the forklift to adjust a pallet by hand. The pallet fell on him, resulting in an injury to the left leg.

An employee was lifting a 500-pound steel counterweight off a pallet using a magnetic hold jib crane. The counterweight detached from the crane and the employee sustained a left foot/toe fracture that required surgery.

An employee was using a pipe wrench to turn a 42-foot-long steel pipe on jack stands to weld the bottom portion. The pipe fell off the stands, striking the injured employee on his left shin. The employee sustained a left leg fracture at the shin area as well as a tibia fracture that required surgery.

An employee was inside a trailer, loading freight, when their leg was caught in shrink wrap. The employee fell, resulting in a fracture to their right thigh bone, which required surgery.

An employee was lowering a safe stack beam inside a trailer. The beam released, causing the employee to fall back and strike his head on the trailer floor. He was hospitalized with a brain bleed and a concussion.

An employee fell from a dock to ground level (less than 4 feet) and broke their tibia/knee.

An employee was rolling up the landing gear on his trailer when the handle began to unwind and struck his face, fracturing his jaw. The employee was hospitalized and required surgery.

On August 20, 2025, an employee was exiting a commercial tractor from the driver's side door when they fell to the ground, resulting in a fractured left ankle.

An employee was traveling down the maintenance aisle of a warehouse on a single-pallet front rider jack. The jack malfunctioned, causing the brakes to apply; the employee fell forward onto the concrete warehouse floor. The employee suffered an injury to the left leg and was hospitalized, requiring surgery.

An employee was operating a metal roller machine to roll a small piece of metal. The employee's right index finger became caught between the metal and the machine, and the fingertip was amputated.

An employee was shot in the leg with a pistol during an altercation with two people who had entered the store without permission.

An employee was directing a truck driver to a dump site when the truck's peanut trailer slipped off the kingpin. As the trailer fell, a ladder attached to it lacerated the employee's ear. The employee was hospitalized.
