An employee was performing preventative maintenance on a drive motor that operates a conveyor belt when his right hand was caught by the belt. He broke his hand and lost several fingertips. The belt was unguarded at the time of the incident.
A delivery driver was making a delivery at a client location. He was closing the rolling door of the delivery truck when his fingertip became caught in the door, resulting in a partial amputation.
An employee was attempting to remove a belt from a conveyor. He used a bar for leverage to remove the belt from the pulley. The motor was on at the time. As the motor caught the bar, it jerked him back and his hand made contact with the other belt, potentially amputating his left middle finger.
