An employee was in the pump room troubleshooting a pump. He unscrewed the top panel of the pump and inadvertently hit the toggle switch that activated the pump. The internal belt caught his right index finger, causing a laceration that resulted in a partial surgical amputation.

On February 11, 2020, an employee was using a hose to spray off equipment. The employee attempted to move a conveyor belt, but its casters were locked; the employee's left hand slipped into the running conveyor belt, which amputated the middle fingertip.

Two employees were troubleshooting a conveyor in a feed mill. The conveyor caught one employee's coat and pulled his left arm in. The arm was lacerated and his index finger was partially amputated. He was hospitalized.

On October 29, 2018, at approximately 6:15 a.m., an employee was standing on the top step of the stairs connected to a truck cab when the step gave way. He fell to the ground, suffering three fractured ribs and a ruptured spleen.

On June 8, 2018, an employee was burned on the face, neck and back after opening a hot oil valve in the browning area.

An employee was operating a machine when a wire pulled his right glove resulting in amputation of the index and middle fingertips.

An employee was grinding chicken when their right index fingertip was caught in the grinder and amputated.

An employee was sharpening peeler blades when their shirt was caught in the grinding wheel. The employee sustained tendon damage and a crushed right hand.

An employee was passing through a large metal door when the door closed on the employee, resulting in a cracked rib.

An employee was preparing to cut lumber on a panel saw when the clamp engaged and caught the employee's left middle finger, resulting in a fingertip amputation.

An employee was operating an agricultural tractor when the tire was punctured and the tractor rolled over onto the employee. The employee was hospitalized with a fractured back and bruising to their heart and lungs.

An employee was filling the tire of a semi-trailer when the tire blew and contacted the employee, resulting in a fractured tibia.

An employee was walking through the kitchen when they slipped and strained their lower back, resulting in hospitalization.

An employee was repairing an attic ladder on the second floor. The ladder opened, and employee fell through to the floor below. The employee was hospitalized with skull fractures, spinal fractures, and a spleen injury.

An employee was servicing a portable conveyor at a site and spun the pulley. The employee's fingertip was caught between the belt and the pulley, resulting in a fingertip amputation.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
