An employee was threading up a paper machine when his hand was caught and pinched by a hydraulic cylinder, lacerating his third, fourth, and fifth fingers and fracturing his fourth and fifth fingers.

Georgia Pacific Consumer Products, LP.

An employee was training a new employee on the operation of a napkin folding machine when he heard a noise that concerned him and asked the trainee to bump the conveyor leading out of the machine. The employee went behind the machine to investigate and slipped as the trainee was bumping the conveyor. His left thumb was subsequently caught between the belt and a timing gear, damaging the bone and tissue. His thumb tip was amputated directly below the nailbed.
