Final narrative
An employee was changing the belts on a pumping unit. The employee left one bolt in place and held the 40-pound guard up with his shoulder. The guard slipped from his shoulder and lacerated his bicep, requiring hospitalization.
An employee was on a workover rig, transferring rods from the transfer and screwing them in, when his glove/sleeve became snagged on the equipment and was twisted around, amputating his left thumb.
A crew was pulling a 4-inch plastic gas pipe off a reel and straightening it for installation.
