An employee was struck by a truck while flagging traffic. The employee suffered multiple fractures to the left shoulder, left leg, right leg, lumbar spine, right wrist, and left arm, as well as tears to the ligaments of their left knee. The employee was hospitalized and required surgery.

An employee was up a tree, trimming tree branches with a chainsaw. The chainsaw cut his lanyard and he fell to the ground about 18 feet below. He suffered fractured vertebrae in his lower back, as well as a punctured bowel, and was hospitalized.

An employee was delimbing a cut-down tree. One of the limbs kicked back and struck the employee between the ankle and the knee, breaking the employee's tibia.

He lost balance and fell backward away from the structure and landed on the ground about 30 feet below. The employee sustained a broken right leg. The employee was wearing a harness but it was not connected at the time of the fall.

An employee fell approximately 30 feet from a roof, resulting in head and spine injuries.

On June 16, 2023, an employee was checking the I-beams on a bridge. He unhooked his harness to move across the section of the second beam. While hooking his harness on the third beam, his foot hit the cable that was connected to the beam and fell approximately 25 to 30 feet to the muddy clay floor below. The employee sustained a left hip and femur fracture.

An employee was using a 60-foot ladder to access the fourth floor of a building in order to paint the balcony. The employee was holding onto the handrail while painting it when the handrail brackets broke, causing him to fall approximately 30 feet to the ground and land on top of air conditioning units. The employee suffered severe lacerations on the face and fractures to the cheekbones and eye bones. The employee was hospitalized.

An employee entered a portable structure that was being rigged up to a crane. The crane began lifting the portable structure with the employee inside when the structure fell 15-30 feet to the ground. The employee sustained a left leg fracture, a dislocated right ankle, and back injury.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was helping to set up a mulch blowing truck when they were struck by an oncoming vehicle. The employee was hospitalized with a fractured knee, a concussion, and a head laceration; they also received stitches on their arm.

An employee was rappelling down a tree. When he was most of the way down, he fell on the ground and suffered a broken pelvis.

An employee was using a table saw to cut a piece of wood when the saw cut his thumb, index, and middle fingers. His index finger was amputated and he was hospitalized.

An employee was mowing a residential lawn at a client's house. The mower slid on the land and tipped over onto him. The employee was hospitalized with a fractured vertebra in the neck and partial paralysis.

An employee was working to deliver mail to an apartment building. She was waiting for a customer to move, to obtain clearance to the mailboxes. The door swung inward and closed on her right little finger. The top half of her finger was surgically amputated.

An employee fell from a step ladder while reaching for a tie-off point. He impacted the ground, and landed on his bolt bag which contained tools. The employee was hospitalized with fractured ribs, and a lacerated spleen and kidney.

On October 30, 2025, an employee was working to adjust a stackable metal shipping container. As the container dropped into place, it caught the employee's hands in an area between the upper and lower cross-members. The employee suffered a laceration to the left ring finger that required stitches, bruising to the right ring finger, and fractures to the right middle finger that necessitated medical amputation of the fingertip.

An employee was moving a scissor lift through a doorway. The employee was pinned between the scissor lift and the doorframe, sustained a back injury, and was hospitalized.

An employee was setting up communication equipment for a meeting. They were walking and tripped over a speaker on the ground. The employee sustained a leg injury.
