An employee was feeding rebar coil into a coil bender machine. He was trying to free a jammed coil from the measuring wheel with a screwdriver when the tool became jammed in the measuring wheel. This caused the coil to snag his glove and drag his thumb and index finger back into the transport wheels. His right index finger was partially amputated.
An employee was using a bench grinder to grind a bevel on a tail piece used in a spike machine. The tail piece got caught and was pulled down to the part rest, causing the employee's right thumb to be pulled into the grinding disc pad. The thumb tip was amputated.

On October 21, 2025, an employee was operating a saw when a pneumatic clamp caught their right middle finger. The employee suffered a partial amputation to the finger.
