While returning from lunch and entering the yard office doorway, an employee tripped on a step and fell, suffering multiple fractures to the right leg and ankle.

An employee was working on a manual clamp. He was holding the clamp with his left hand and holding a cylinder with his right hand. His right index finger was inside a slot on the cylinder housing when the clamp arm slipped out of his left hand and fell. When the unit fell, his right index finger was still in the slot and the top portion of the cylinder came down and amputated his finger. The equipment was not locked out at the time of the incident.

The employee was putting sticks onto a conveyor belt when his right hand and arm got caught in the conveyor resulting in three broken fingers, a fractured wrist and abrasions on the arm.

An employee was unloading a trailer when a box fell and struck the employee in the face. The employee suffered a concussion and was hospitalized.

An employee had been cutting lumber for a shed wall. A load of 16-foot trim pieces fell onto the employee causing vertebrae and ankle fractures.

An employee was unloading 30-foot 2-inch barrels from a pan holding about 20 barrels. The barrels tipped over, striking and breaking both of the employee's lower legs.

An employee was in a sandblasting area, preparing the surface of a plate heat exchanger frame for painting. The frame fell on the employee, who suffered fractures to vertebrae, the neck, pelvis, and rib cage; and punctured lungs.

An employee was installing doors when one of the doors fell on him, fracturing his lower leg. The employee was hospitalized.

An employee had made adjustments inside a wood moulder and was operating the machine when they sustained a left ring fingertip amputation as well as a laceration to the middle finger.

An employee was changing out floor paper under a chain conveyor inside a topcoat cure oven. His left middle finger was pulled into a pinch point between the conveyor's chain and a gear. The skin of the fingertip was degloved, and the fingertip was medically amputated at the first joint.

Two employees were preparing to pour concrete to manufacture burial vaults. The belt on the concrete auger broke. One of the employees had replaced the belt and was fixing a twist in it, with his left middle finger under it, when the machine was turned on. The fingertip was amputated about 1/4 inch below the nail.

An employee was closing a door to a paint booth. The employee's left thumb was caught between the door and the jamb, sustaining a partial amputation to the tip.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
