An employee was assisting a hoist operator and guiding a plate down onto a pallet. While the plate was rested onto the pallet and released from the rigging, it shifted, pinching the employee's middle fingertip between the floor and the plate. The employee's fingertip was amputated.

An employee was crossing over a sorting belt and jumped down onto the catwalk next to the belt. The employee sustained a right ankle fracture and dislocation.

On November 7, 2023, at 6:00 PM, an employee was getting off a roof at the end of the work day. He missed a step and then jumped from the ladder to the ground sustaining a fractured ankle.

An employee was on the table of a machine. She went to jump down from the table and fractured her lower leg on the ground. The employee was hospitalized.

An employee was loading a flatbed trailer when a forklift bumped into the trailer. A wheelset began to roll in his direction and the employee jumped off the trailer to avoid it. The wheelset caught him on the leg and he landed on his shoulder on the ground. He sustained a laceration below the left kneecap and a dislocated shoulder.

An employee jumped off the loading dock to assist a package delivery driver who fell. He sustained an ankle fracture that required surgery.

An employee was preparing to remove the drive chain of a conveyor motor. Their thumb became caught in the sprocket and chain of the drive motor, resulting in an amputation to the thumb tip between the nail and the first joint.

An employee was working with a drill press when his glove and forearm were caught by the drill bit, resulting in lacerations and fractures to his right forearm. The employee's arm was amputated below the elbow.

An employee was walking on a sidewalk when he stepped off the sidewalk into the grass. As he was stepping onto the concrete parking lot, he tripped over the edge of the concrete and fell onto his left hip. The employee was hospitalized.

An employee was changing the cooling nozzles on an aftercooler and the pneumatic power was activated. This caused the top plate to move into the top position and crush the employee's right arm. The employee sustained a fractured arm.

On October 15, 2024, an employee was cleaning a drum welder with a rag. The machine pulled in the rag and injured three fingers on the employee's left hand. The employee was hospitalized with three partially amputated fingers.

The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.

An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.

A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.

An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.

An employee was unloading a truck for a delivery job. While moving boxes in the truck, the employee tripped over a pallet and fell out of the truck to the ground, resulting in four broken ribs.
