An employee was working in a walk-in freezer when a powered industrial truck hit and dragged him against an upright beam. He suffered a contusion to his lower back and was hospitalized.

On February 26, 2017, an employee was driving a forklift and was turning around in a small area. There was another forklift that was non-operable in the same area. The employee backed up the forklift and struck the forks of the non-operable forklift. The forks penetrated the employee's right heel and tore tissue from the bone.

An employee tripped over a customer U-Boat and sustained blunt trauma to the back of his head and pain to his right shoulder.

An employee was hospitalized for chemical burn to the eyes.

An employee was preparing a tank car for loading while chipping away solidified product in the manway opening of the tank car. A pressure noise within the tank car startled the employee, causing him to stumble backward and fall approximately 10 feet over the guard railing at the chemical loading rack to the ground. The employee suffered fractured vertebrae, a fractured right wrist and ankle, and a dislocated jaw.

An employee was installing an electrical tray remover from a ladder. As he was descending, his missed a step and fell 6-7 feet to the floor. The employee suffered a head injury.

Two employees were installing spray foam insulation in the attic of a new residential construction. The injured employee fell 8 feet from an A-frame ladder to the floor. The employee suffered lacerations, abrasions, and a head injury.

An employee was on a raised platform attaching wires to electrical boxes. The employee fell approximately 8 feet to the floor below and struck an exit sign on the way down, resulting in an injury to his face.

An employee was in a forklift man basket fixing a garage door opener when they fell approximately 8 feet to the concrete ground below. The employee suffered head and back injuries as well as kidney bruising.

An employee tripped over a bin and fell to the floor. She landed on her hip and braced herself with her arm, resulting in a fractured hip and wrist.

An employee was reaching for a piece of angle iron when it fell onto his hand, which was on a table to support him as he reached for the overhead rack. He suffered a broken finger and was hospitalized, requiring surgery.

An employee was putting dough into a forming machine when their left little fingertip was amputated by the forming die.

An employee slipped and fell outside, landing on her hip on concrete. The hip was broken and she was hospitalized, requiring surgery.

An employee was working to open the liftgate on the back of a delivery truck. His left middle finger was pinched between the gate and its frame, resulting in a fingertip amputation.

An employee was cleaning leaves with a backpack blower when he fell from a ladder, resulting in injuries to his head, neck, and back.

An employee was operating a battery-powered broom to clean demolition debris off the top of the outermost bridge girder. The girder was approximately 11 feet above the demolition scaffold below. The employee fell approximately 8 feet to wind bracing below. The employee was hospitalized with a pelvis/tailbone fracture. Fall protection was in place at the time.

An employee slipped while descending in a stairwell, fell, and suffered multiple contusions/bruises to the head, neck, shoulders, knees, and back. She was hospitalized.

An employee's right middle finger was caught in a nip point. The employee suffered an amputation to the middle finger at the first knuckle.

An employee was directing trucks as they entered and left a highway work zone. A vehicle struck them, resulting in multiple fractures and other injuries. The employee was hospitalized.
