An employee was unjamming a board on the planer mill. A 14' poplar board came off the planer mill belt and pinned the employee's left hand between the board and the barrier guard causing an amputation of the left index and middle fingertips.

An employee was tossing a damaged toilet into a compactor. The toilet bounced off the compactor and lacerated the employee's right wrist, cutting an artery and tendon.

An employee was operating a hydraulic jack to bend a frame when the jack came loose and struck the employee. They were hospitalized for a spleen injury.

RMVK Enterprise, INC.

A truck had been towed into the shop with a flat tire and a blown airbag. An employee checked the tire for damage and was airing the tire. The employee was installing the valve core when the bead slipped off the rim. Air escaped, and the aluminum wheel struck the employee's head and face. The employee became unresponsive and was bleeding.

Dustrol Inc.

On December 14, 2023, at 8:40 p.m., three employees were inspecting milling machines for startup. A few of the sleeves on the mill drum needed to be changed out. The injured employee was trying to input a sleeve on the mill drum using a hammer. The hammer hit the edge of the drum, which caused a metal piece to splinter off and strike the employee's cheek just below the left eye. The metal splinter embedded in the back of their eye socket, resulting in pain, swelling, and bleeding that required hospitalization.

An employee was pushing a line of carts into a vestibule when the door cable snapped and struck his right ankle. The employee suffered a fracture that required surgery and hospitalization.

Interlake Mecalux, Inc.

On August 13, 2025, an employee was feeding wire through a junction box to feed a 120-volt circuit near a 480-volt bus duct. The bus duct suddenly combusted, and the employee suffered burns to his left hand and upper body. He was hospitalized.

New England Wire Products Inc.

An employee was operating a brake press and making the final bend on a steel part. Her right middle finger got caught between the part and the top punch as it bent the part upward. The employee sustained a crush injury resulting in amputation of the fingertip.

An employee was feeding scrap metal into a chipper when his right hand was squeezed against a metal component of the machine. He suffered an amputation to the right index finger at the second joint.

An employee was setting up a press brake when the machine was activated and her left middle fingertip was caught between the dies and amputated.

On June 18, 2025, an employee working as a millwork installer was cutting shims using a circular saw. The saw hit a knot in the wood and jumped, cutting the palm of the employee's left hand and causing partial amputations to three fingers. The employee was hospitalized.

An employee was checking a pole-type transformer tank for leaks. A hydraulic clamping device was lowered onto his right thumb and crushed it against the sharp edge of the bottom of the tank. He suffered a partial transphalangeal amputation to the thumb.

The injured employee was standing behind a table. She jumped out to scare another employee, but tripped and fell on the tile floor, injuring her right ankle/foot. She was hospitalized with a dislocated ankle that required surgery.

An employee was going into a trailer to mark product for shipment and the trailer pulled away from the dock. The employee fell out of the trailer, contacted the dock plate, and then fell to the ground. The employee was hospitalized with a punctured lung, fractured ribs, and contusions.

Ingalls Shipbuilding

An employee was hooking up bundled tie-downs with a chain. While he was holding a hook, the other hook was unlocked. This caused the employee's hook to slide down and pinch his right index finger between the chain and the shackle. He suffered an amputation to the fingertip (without bone loss), as well as an open fracture.

An employee was operating a gas tugger to lift metal sheeting. His left hand was pulled into the pulley, which crushed his left thumb, resulting in avulsions and other tissue damage. He was hospitalized and required surgery.
