Final narrative
An employee accessed the facility roof to conduct preventive maintenance inspections on exhaust fans. He went to ascend a fixed ladder to access a lower portion of the roof. He was standing on the second or third rung when the ladder extension on the top of the ladder failed and he fell approximately 12 feet to the roof level below. He was hospitalized for back and leg pain.
