On October 18, 2016, at 4:15 p.m. a temporary employee was caught between the metal building insulation (MBI) machine and the push off bar on the machine while reaching over the machine to pull insulation. The employee sustained a fractured pelvis and was hospitalized.

An employee was operating the laminator machine and was trying to move material through the winding bar, but the material would not roll through. When he attempted to pull the material through, his arm was caught in the winding bar. He required surgery on his right forearm.
