Final narrative
An employee had just unjammed a roll former and was running another flat piece through the rollers to try and determine what had caused the jam. The flat metal piece became caught in the rollers. The employee was trying to pull it through with his hands when his gloves were caught and both of his hands were pulled between the rollers. Two of his right-hand fingertips were amputated, and four left fingers were smashed. The machine was unguarded at the time.
On 8/18/2017, an employee was operating a sheet metal electric power rotary beading and crimping machine to make collars. The employee's leather glove was caught between the roller and the crimping of the machine wheel, and the left index finger was amputated.
Two employees were repairing the hydraulic engagement pins on a rented front-end wheel loader. The machine controls were activated to move the attachment pins and an employee's finger was caught between the pins and the bushings. Their right index finger was partially amputated.
