Final narrative
On November 4, 2016, an employee was moving laminated beams when one fell, hit the ground, bounced back up, and pinched the employee's right middle fingertip, resulting in an amputation.
An employee was unloading trusses from a trailer. The trusses slid off the side of the trailer and struck the employee, who suffered fractures to the ribs, femurs, left hip, and left arm.
An employee was using a V-nailer to staple a door casing together and used his right hand to line the casing up and hold the casing in place. The foot pedal was activated and the clamp came down on his right little finger amputating the fingertip just below the fingernail.
An employee was standing on the ground beside his tractor trailer and strapping down his load. A forklift crushed him against the trailer as it backed past him. He suffered a broken pelvis and internal bleeding.
An employee was climbing an 8-foot step ladder and stopped at the second step from the top to secure a beam that a forklift was placing in the building. The beam then fell toward the employee, causing him to fall backward and hit his head on the concrete slab below. The employee suffered a fractured skull.

An employee was walking down the off ramp from a box truck, carrying a door for delivery. A strong wind caught the door, causing the employee to lose balance and fall about 2 to 3 feet to the ground. He landed on his back and suffered a broken vertebra.
An employee was delivering lumber to a customer's job site. He was using a truck-mounted forklift to transport the material to the staging area. The surface of the path of travel changed from asphalt to soil and rock with a slight downward grade. When the forklift moved onto the softer surface, it sank into the ground and its center of gravity shifted forward, causing it to tip over. The employee's right arm sustained a closed, supracondylar humerus fracture.
An employee was helping two forklift operators move and inventory flat sheet metal that was staged on racks. The metal fell off one side and struck the employee's arms and abdomen. The employee sustained a laceration above the left eye, a left forearm injury, and a ruptured spleen. The employee was hospitalized and required surgery.
The injured employee was assisting five other employees with bracing a wall panel that was being stood up. Another employee slipped on frost on the floor surface. The employees went to evacuate the area in anticipation of the wall panel falling over. The injured employee went to jump through a window opening within the wall panel and their lower half was crushed by the falling wall panel. The employee sustained a fractured pelvis.
An employee was using a vertical band saw to cut a thick piece of rubber material. While being cut, the material shifted, causing the employee s right hand to contact the moving blade. The employee's thumb was amputated below the knuckle. The employee was hospitalized, undergoing surgery.
A production sanitation employee was pre-rinsing an incline conveyor and noticed cheese stuck between the belt idler. The employee went to remove the cheese and their glove got caught and pulled their left forearm into the conveyor. The employee's arm was fractured. The machine was not locked out/tagged out at the time.
An employee was on a ladder, taking measurements for a new gutter installation. The ladder slipped out from under him and both he and the ladder fell to the ground. He was hospitalized with seven broken ribs and a broken clavicle.
