An employee was cleaning a walkway with a compressed air hose when he tripped over the hose and fell to the ground. As the employee fell to the ground, his right hand landed on the belt of a conveyor system. His hand was then pulled into the area of the conveyor where the belt turns over the drum, and his right hand was caught between the belt, drum, and frame of the conveyor. He sustained a laceration and burns to his right hand and wrist.

An employee was unloading a trailer when a box fell and struck the employee in the face. The employee suffered a concussion and was hospitalized.

An employee had been cutting lumber for a shed wall. A load of 16-foot trim pieces fell onto the employee causing vertebrae and ankle fractures.

The injured employee was standing on a ladder performing this task when they fell feet-first into the hydraulic oil pit, resulting in burns on both legs.

An employee slipped on condensation on a walkway in a parking garage. She fell and suffered a closed fracture to the neck of the left femur.

An employee was using a tool to remove a rag from a roll on the tube mill. The roll pulled the tool and the employee's right hand into the roll, resulting in a partial amputation of the little finger and a fracture to the index finger.
